Alternatively, anyone living outwith PA1, 2 or 3 and make a movement to another area, or same postcode, will be asked to leave our practice list. ie living in PA4 and move to PA5 or living in PA5 and move to another house in PA5.

How to Register as a Patient

New patients wishing to join our practice are given an appointment with the Practice Nurse for a Registration Medical. You will not see a GP at this time.

No medication will be issued at this appointment and you should therefore ensure that you have enough medication to last you from your current GP surgery.

Information Required

  • You must provide us with 2 forms of personal identification, one of which should be photographic (i.e. passport, driving licence). Please ensure you bring these with you to your registration medical as failure to do so will result in your registration process being delayed.
  • Please download and complete the forms:

These forms are also available from reception. Please fill in as much information as possible on the registration forms before you come to the appointment.

Non-urgent advice: Notice

Please Note

It is our practice policy to allocate new patients a registration appointment with the Practice Nurse. If you know you cannot attend your registration appointment, you should telephone us on 0141 889 8809 as soon as possible to allow the appointments to be given to other patients and to reschedule a more suitable time/date.
